What is amputation?

What is amputation?

Tooth amputation in children is a conservative treatment method applied to primary or newly erupted permanent teeth. Instead of extracting the tooth completely, it aims to preserve the tooth in the mouth by removing only the problematic part (usually the nerve or root area). In pedodontics, this approach is generally applied as root interventions as an alternative to pulpotomy or pulpectomy. Basic Principle

The main goal in pediatric dentistry:

* Keep the tooth in the mouth for as long as possible

* To continue chewing, speaking, and holding space.

Therefore, unlike in adults, the tooth is not extracted immediately; only the problematic part is removed, and the healthy part is preserved.

In what situations is it preferred?

* Nerve damage due to advanced decay

* Damage to nerve tissue following trauma

* Infection in one of the roots of a baby molar tooth

* Situations where root canal treatment cannot be fully performed

* Cases where it is important for the tooth to remain in the mouth instead of being extracted;

Procedures Performed (Types of Amputation in Children)

1. Pulpotomy (the most common procedure)

* The nerve tissue at the top of the tooth is cleaned.

* Healthy tissue in the roots is preserved.

* It maintains the vitality of the tooth.

* In other words, it is the most common "partial removal procedure" performed on children.

2. Pulpectomy (in more advanced cases)

* All nerve tissue is removed.

* The insides of the canals are filled.

* Preferred if the infection is advanced.

3. Root amputation (less common)

* Applied to primary teeth with multiple roots.

* Only the problematic root is removed.

* The rest of the tooth is kept in the mouth.

The sequence of operations;

  1. First, an examination and X-ray are performed.
  2. Local anesthesia is administered (the child will not feel pain).
  3. Decayed tissues are removed.
  4. The diseased pulp or root portion is removed.
  5. Therapeutic materials are applied.
  6. The tooth is sealed (often supported by a steel crown).

The recovery process;

* It usually progresses quickly and smoothly.

* Mild sensitivity may occur during the first 1-2 days.

* The child will soon return to normal life.

Why is this so important?

Because baby teeth:

* It reserves space for the permanent teeth that will erupt from below.

* If extracted prematurely, it can cause teeth to shift and become misaligned.

* Chewing and speaking are affected.

Therefore, amputation is a much more conservative and intelligent approach than extraction.

Things to keep in mind:

* Oral hygiene should be carefully observed.

* Regular dental check-ups should not be neglected.

* The treated tooth should be monitored.

Briefly

Tooth amputation in children can be defined as "a procedure that saves the tooth by removing only the problematic part, without losing the entire tooth.".
